Learn from mistakes - do not be defensive
When something goes wrong, the reaction of most people is defensive. They immediately look for evidence to prove that it is not their fault. 

I prefer to take a different approach.

I want to see what went wrong and how the error can be rectified. It does not matter to me who was responsible for the fault, because it is likely to be accidental and unforeseen.

We can learn from the mistake and look for the way to prevent its future occurrence. 

Mistakes and accidents are part of daily lives. There is no need to be defensive. 

However, we should be diligent to learn from the mistake and avoid them in the future. 

A defensive attitude does not help to identify the mistake. Often, by focusing on blaming other people, we miss the change of getting to the root of the problem and learning from the mistake.

The defensive attitude is bad, but it is quite common.
